We are excited to welcome you to MHCA! Please note that only members of the Midvale Heights neighborhood are eligible to join. Please use the city of Madison neighborhood map to plug in your address and verify you live within our boundaries. Need help? Use our contact form and thank you!  

Membership Form - Two Ways to Join or Renew

MHCA is a registered 501(c)3 non-profit, so your membership and any additional donations are tax deductible.

  • Prefer to mail a check? Download our printable membership form.


As we print a member directory in October, distributed only to other members, fill in details for all adults and children in the household. MHCA does not share or sell this information, and you may opt out of being in the directory if you like while still receiving a copy. If you prefer to leave just your children out, there is a place to note that, or skip that portion of the form.

PLEASE NOTE: MHCA membership is only open to residents of Midvale Heights. Please review the map to ensure you are eligible. Incorrect payments will be converted to donations and will not be refunded.
