The form below allows you to check out via PayPal with a credit card. You do not need a PayPal account. If you prefer to mail a check, please download our printable membership form.

As we print a member directory, distributed only to other members, fill in details for all adults or children in the household. MHCA does not share or sell this information, and you may opt out of being in the directory if you like while still receiving a copy.
